Step 2: Then, when you are contacted by a member of our team, you will need to contact one of the Head Start or Early Head Start locations to schedule an appointment. This is where the entire application process will be completed.

Step 3: Please bring the following information / documentation to your application appointment:

  • Child’s Birth Certificate
  • Social Security Card
  • Current Utility Bill as Proof of Residency
  • Proof of Income (W-2, Income Tax Return or Budget Sheet)
  • Medical Card
  • Immunization Record
